Background: Endometriosis is a benign gynecologic condition that has the risk of malignant transformation in approximately 0.5–1% cases, most which develop into endometriosis-associated ovarian cancers (EAOCs), such as clear cell and endometrioid adenocarcinomas. The current systematic review aims to condense recent information on genetic molecular mechanisms, clinical factors, possible therapeutic targets endometriosis. Methods: A literature search PubMed, Europe PMC, Google Scholar was carried out according PRISMA guidelines for articles published until December 2024. Following screening 44,629 titles, 43 full were included after meeting inclusion criteria. No case reports or reviews included, had mention endometriosis not just diagnosis cancer. quality bias studies evaluated using ROBINS-I. Results: Malignant associated with alterations, including ARID1A mutations, microsatellite instability, abnormal PI3K/Akt mTOR pathway activation. Increased oxidative stress, inflammation-driven mismatch repair deficiency, epigenetic alterations like RUNX3 RASSF2 hypermethylation are implicated carcinogenesis. Clinical factors advanced age (40–60 years), large endometriomas (>9 cm), postmenopausal status, prolonged estrogen exposure. Imaging techniques MR relaxometry models based machine learning highly predictive early detection. Conclusions: carcinogenesis multifactorial process driven by changes, inflammatory mechanisms. Identification high-risk individuals through imaging biomarkers may result detection personalized therapy. Further research should aim at development more precise exploration precision medicine approaches inhibit emergence EAOC.
