Rapid or semi-rapid maxillary expansion is a well-established method of treating severe transversal deficiency dental arch discrepancy. Pain considered an inevitable undesirable adverse effect orthodontic treatment. The aim the present study was to compare pain levels experienced from semi-RME (SRME) procedure between adults and adolescents lower fixed appliance (FA) in same patients. comprised 95 patients (51 44 adolescents) treated with bonded acrylic Hass-type expander following month (after blocking screw) appliance. assessed using numerical rating scale 0 10. During screw activation, adolescent reported statistically significantly higher median level (5.00) compared (3.00) [p = 0.009]. treatment, similar (4.5 4.0, respectively; p 0.49). Interestingly, for SRME whole group 4, not different treatment sensations. Adolescent females had highest sensation, while adult males lowest during both procedures, FA. difference midpalatal suture opening (appearance diastema) (81.82%) vs. (64.71%) significant (p 0.062). oldest diastema were 36y-2mo-old man 42-year-old woman, whereas youngest without 11y-11mo-old girl 13y-2mo-old boy. Moreover, all female 30 42 years age (eight women) presented diastema. In groups, biggest disadvantage SRME, by 50.53% (45.45% 54.9% adults; 0.062), speech problems, only 18.95% total (27.27% 11.76% 0.054) as main disadvantage. FA therapy, 32.63% pressure sores (29.55% 35.29% 0.551).

Multiple sensory afferents, including mechanosensitive and nociceptive nerves, are projected to the periodontium. Peptidergic afferents expressing transient receptor potential vanilloid 1 (TRPV1), a for capsaicin, mediate pain caused by orthodontic forces. However, their role in force-induced alveolar bone remodeling is poorly understood as contribution of ion channels such Piezo2 nerves. To investigate this role, we studied tooth movement using neural manipulations genetic mouse models. Chemical ablation TRPV1-expressing localized trigeminal ganglia decreased force–induced number osteoclasts on compression side. The extent increase ratio activator nuclear factor kappa-B ligand/osteoprotegerin periodontium was modestly chemical group. Furthermore, chemogenetic silencing TRPV1-lineage reduced osteoclasts. expressed most periodontal inhibition Piezo2-expressing neurons In addition, conditional knockout Overall, these results suggest that nociceptor play critical roles mechanical activation neuronal nerves facilitates associated remodeling.

P2X receptors (P2XRs), membrane ion channels activated by extracellular adenosine 5' -triphosphate (ATP), play a pivotal role in nociception directly promoting pain signaling. Currently, antagonists targeting P2XRs have taken effect alleviating various pain. The therapeutic potential of the receptor family has become central focus. Consequently, numerous research groups and pharmaceutical companies are actively engaged developing novel P2XR antagonists. Furthermore, an increasing number clinical trials on obtained encouraging results. This review provides overview structural characteristics cellular localization P2XRs, their molecular mechanisms signaling pathways implicated orofacial Additionally, it explores development application for managing In conclusion, this highlights promising as targets treatment.

Orthodontic tooth movement (OTM) induces physiological and sometimes pathological inflammation in periodontal tissues. This review evaluates the effectiveness of low-level laser therapy (LLLT), vibrational therapy, probiotics as adjunctive treatments for managing inflammation, pain, duration OTM. Medline via OVID, Cochrane, EMBASE, Web Science databases were utilized to identify randomized controlled trials (RCTs) published between January 1990 November 2023. Studies selected based on their evaluation LLLT, adjuncts fixed orthodontic treatment. LLLT shows promise enhancing by accelerating potentially reducing pain. However, disparate study outcomes indicate a need standardized application protocols. The efficacy an adjunct OTM remains inconclusive. Some studies this regard significant acceleration but most did not. Probiotic potential improve oral microbiota balance requires more rigorous determine its optimal administration methods. Future research should focus establishing guidelines protocols achieve consistent reliable across these therapies.
