Abstract Background The increasing prevalence of mental health disorders in the United Kingdom necessitates exploration novel treatment modalities. This study aimed to assess feasibility and acceptability conducting a randomised controlled trial (RCT) evaluating efficacy four osteopathic interventions on psychophysiological outcomes. Methods A mixed-methods with an explanatory sequential design was implemented. quantitative phase involved randomising 42 participants into intervention groups: (1) high-velocity articulation techniques (HVAT), (2) soft-tissue massage (STM), (3) craniosacral therapy (CST), (4) combination approach. Primary outcome measures encompassed recruitment rate, assessment duration, questionnaire completion, attrition, adverse events. Secondary outcomes included validated assessments depression, anxiety, stress, psychological flexibility, heart rate variability (HRV), interoception, administered pre- post-intervention. Analysis variance (ANOVA) employed evaluate pre-post changes. qualitative comprised semi-structured interviews analysed using thematic analysis. Results achieved 21 eligible per month, 54.8% respondents meeting eligibility criteria. All 33 who completed underwent within allocated one-hour timeframe, full completion. attrition 21%. No events were reported. Qualitative analysis revealed positive participant experiences, themes highlighting good practitioner communication, accessibility, increased bodily awareness. Some found battery burdensome. Exploratory analyses showed variations effects across for variability, interoceptive accuracy, measures, but these results should be interpreted cautiously due small sample size. Conclusions provides evidence supporting larger-scale RCT investigating individuals presenting mild symptoms. preliminary findings suggest potential improving outcomes, warranting further investigation. Trial registration NCT05674071, registered 06/01/2023.
