Polycystic ovary syndrome (PCOS) is a common endocrine and metabolic disorder in women of reproductive age. Obesity has significant implications for fertility and reproductive health. However, evidences linking abdominal obesity to female infertility were limited inconclusive. Our objective was figure out the potential relationship between waist circumference (WC) among women of childbearing age in United States using data from National Health Nutrition Examination Survey (NHANES).Our cross-sectional study included 3239 participants aged 18-45 years. To explore independent WC infertility, weighted multivariable logistic regression smoothed curve fitting performed. Interaction subgroup analyzes then conducted secondary analysis.WC positively associated with BMI after adjusting other confounders. In fully adjusted model, every 1cm increase circumference, risk increased by 3% (OR = 1.03, 95% CI: 1.01-1.06). Gravidity may influence psychological outcomes, with nulligravida women facing fertility-related distress, primigravida experiencing pregnancy-related anxiety, multigravida dealing cumulative stress worsening PCOS symptoms. This study aims to compare depression, stress, quality of life (QoL) among these groups inform targeted, stage-specific interventions improve well-being in PCOS.
