Objective This study aims to assess the predictive potential of circulating tumor cells (CTCs) and stem (CTSCs) in locally advanced triple-negative breast cancer (TNBC) undergoing neoadjuvant chemotherapy (NAC) compared RECIST 1.1 standard. Methods We analyzed 112 patients with TNBC at Liaoning Tumor Hospital. CTCs CTSCs were evaluated before NAC, on first NAC cycle day, after NAC. assessed ability positive predict resistance (requiring regimen change) a 91% specificity. Additionally, we CTC dynamics during efficacy (often reaching MP4 or MP5) 87% sensitivity 80% Results Positive post-first predicted high specificity (91%). The gradual decline indicated efficacy, allowing continue 80%. Conclusion suggests that resistance, thereby facilitating early detection (≥ 6 weeks earlier than RECIST). Gradual reduction predicts enabling continuation. show promise as markers for TNBC.
