In the era of digital transformation, utilisation Virtual Reality (VR) applications in brain injury rehabilitation presents a novel frontier with vast potential. However, concerns persist about how traditional communication pathways between therapists and patients can be transformed into equivalents. Immersive virtual reality submerses world, taking role external spectators. This has implications for therapeutic alliance (TA), which is based on interpersonal bonds effective therapist patient. study investigated use immersive VR clinical settings acquired (ABI). The aim was to understand design smart system that adapted various needs during intervention. Through observation semi-structured interviews, patient-therapist interactions were analysed VR-assisted sessions, revealing challenges related collaboration. Findings indicated changes interaction introduction VR: informal dialogue decreased, while shared laughter feedback increased. Despite visual disconnect imposed by headsets, continued guiding gestures engaged patients. Interviews highlighted importance place rapport-building, both relied each other expertise reassurance. research offers valuable insights potential transit, transform enhance practices suggests avenues human-centred tailored ABI TA, ultimately enhancing outcomes this domain.
