Diabetic retinopathy (DR), as one of the most common and significant microvascular complications diabetes mellitus (DM), continues to elude effective targeted treatment for vision loss despite ongoing enrichment understanding its pathogenic mechanisms from perspectives such inflammation oxidative stress. Recent studies have indicated that characteristic neuroglial degeneration induced by DM occurs before onset apparent lesions. In order comprehensively grasp early-stage pathological changes DR, retinal neurovascular unit (NVU) will become a crucial focal point future research into occurrence progression DR. Based on existing evidence, ferroptosis, form cell death regulated processes like ferritinophagy chaperone-mediated autophagy, mediates apoptosis in NVU components, including pericytes ganglion cells. Autophagy-dependent ferroptosis-related factors, BECN1 FABP4, may serve both biomarkers DR development potentially targets treatments.
