Steroid-responsive encephalopathy associated with autoimmune thyroiditis (SREAT) is a rare condition characterised by neuropsychiatric symptoms, presence of antithyroid antibodies and significant response to steroid therapy. Postpartum psychosis (PP), although having low prevalence, psychiatrical emergency potentially serious impact in the mother children. PP has an atypical presentation when compared affective or psychotic episodes unrelated pregnancy. Autoimmune dysfunction frequent postpartum period closely related PP. We report case primiparous woman her 20s who did not respond initial treatment antipsychotics. After reassessment, SREAT was considered differential diagnosis. Neuropsychiatric symptoms improved dramatically after 72 hours high-dose steroids patient discharged 16 days. In women PP, cause must be ruled out before assuming psychiatry aetiology.
