Prostate cancer (PCa) is the most prevalent malignancy and second leading cause of cancer-related deaths among men. While adenocarcinoma prostate (adeno-PCa) well-characterized, neuroendocrine (NEPC) remains poorly understood. Generally, NEPC a rare but highly aggressive histological variant, however its limited patho-physiological understanding leads to insufficient treatment options associated with low survival rates for patients. Current treatments NEPC, including platinum-based therapies, offer some efficacy, there significant need more targeted approaches. This review summarizes molecular characteristics in contrast adeno-PCa, providing comprehensive comparison. A portion discussion dedicated tumor microenvironment (TME), which has recently been identified as key factor progression. The TME includes various cells, signaling molecules, extracellular matrix surrounding tumor, all play critical roles development response treatment. Understanding TME's influence on could uncover new avenues innovative strategies, potentially improving outcomes patients this challenging variant PCa.
