Spindle cell lipoma is a relatively rare benign tumor that can occur in the posterior neck, shoulder, and upper back. Herein, we present case of intrascrotal spindle 71-year-old male who presented with mass left scrotum had developed over 2 years. Contrast-enhanced computed tomography (CT) revealed 5.7cm accompanying enhanced solid components. Magnetic resonance imaging (MRI) showed heterogeneous signal intensity. Therefore, well-differentiated liposarcoma derived from spermatic cord was suspected; therefore, patient underwent radical inguinal orchidectomy high ligation cord. Histopathological examination mature adipocytes bland-spindle cells. Immunohistochemically, cells were positive for CD-34 negative CDK4, MDM2, p16. These findings indicated lipoma. Surgical margins negative. Three months after surgery, no relapse observed. This underscores rarity para-testicular While CT MRI play crucial roles disease diagnosis, they may not detect all lesions. To prevent overtreatment, it’s essential to also consider pre-surgical examinations intraoperative findings.
