Metaplastic thymoma is a rare biphasic thymic tumor with indolent behavior and recurrent YAP1::MAML2 gene rearrangement. Although the diagnosis of this usually straightforward based on hematoxylin eosin (H&E) findings alone, cases scant spindle-cell ("pseudosarcomatous stroma") components can be easily confused more commonly occurring type A thymoma. We present case metaplastic sparse stroma-like component, discussing its histological immunohistochemical hints drawing attention to visual similarity This also first published associated psoriasis.
