Background: Liver transplantation (LT) is regarded as a curative approach for patients with hepatocellular carcinoma (HCC), especially those underlying advanced liver disease. However, the recurrence of HCC post-LT poses significant challenges, reported rates 15-20% within first two years following surgery. Effective management single-nodule critical to improving patient outcomes. Methods: This meta-analysis evaluates efficacy surgical resection versus locoregional therapies (LRT) in localized after LT. We adhered PRISMA Statement conducting thorough search relevant studies published from 2009 2024, ultimately including ten that met our eligibility criteria. Results: The results indicate undergoing treatment displayed superior one-year overall survival (OS) compared receiving LRT (71% vs. 62%, p = 0.038), well higher disease-free (DFS) (60% 54%, 0.042). Notably, group presented more characteristics prior transplantation, microvascular invasion and elevated alpha-fetoprotein levels. Conclusions: Our findings suggest while associated better outcomes, choice between approaches must be individualized based on tumor function. ongoing development standardized guidelines inclusion immunotherapy or targeted agents will essential refining pathways outcomes experiencing
