Background/Objectives: The methylation status of the O6-methylguanine-DNA methyltransferase (MGMT) promoter in gliomas has emerged as a critical biomarker for prognosis and treatment response. Conventional methods assessing MGMT methylation, such methylation-specific PCR, are invasive require tissue sampling. Methods: A comprehensive literature search was performed compliance with updated PRISMA 2020 guidelines within electronic databases MEDLINE/PubMed, Scopus, IEEE Xplore. Search terms, including “MGMT”, “methylation”, “glioma”, “glioblastoma”, “machine learning”, “deep “radiomics”, were adopted various MeSH combinations. Original studies English, Italian, German, French languages considered inclusion. Results: This review analyzed 34 conducted last six years, focusing on using radiomics (RD), deep learning (DL), or combined approaches. These utilized radiological data from public (e.g., BraTS, TCGA) private institutional datasets. Sixteen focused exclusively glioblastoma (GBM), while others included low- high-grade gliomas. Twenty-seven reported diagnostic accuracy, fourteen achieving values above 80%. use DL RD generally resulted higher sensitivity, specificity, although some lower minimum accuracy compared to single model.
