Early detection of aggressive pituitary neuroendocrine tumors (PitNETs) remains challenging due to the absence reliable markers that can predict disease progression. Aggressive are typically identified through long-term observation. Tumor immune microenvironment (TIME) is crucial for understanding PitNETs' heterogeneity and identifying potential predictors tumor aggressiveness. Invasiveness is a major predictor of surgical outcome and long-term prognosis in patients with pituitary adenomas (PAs). We assessed PA invasiveness via radiological, histological perspectives to establish classification scheme for predicting invasive behavior poor prognosis. retrospectively analyzed 903 who underwent transnasal-transsphenoidal surgery between January 2013 December 2019. Radiological (hazard ratio (HR) 5.11, 95% confidence interval (CI): 3.98–6.57, p < 0.001) (HR 6.40, CI: 5.09–8.06, better predicted gross-total resection (GTR) recurrence/progression-free survival (RPFS) rates than did 1.44, 1.14–1.81, = 0.003). Knosp grades 2 4.63, 2.13–10.06, 3 2.23, 1.39–3.59, 0.011) were predictors corresponding without invasiveness. Classifications 1 established based on invasiveness, respectively. Background: Long-term functional and tumor outcomes in patients undergoing endoscopic surgery for non-functioning pituitary adenomas (NFPA) are not well documented. Objective: To study factors influencing the extent of resection (EOR) long-term visual, hormonal, following uni-nostril with nonfunctioning adenomas. Methods: In total 172 patients, ≥1 year follow-up, operated between 2005 2018 were included this retrospective study. Risk analyzed, logistic regression analysis, EOR visual hormonal outcomes. Recurrence rates correlated adjuvant radiation therapy (RT). Results: Gross (GTR) or near (NTR) was achieved 120 (69.7%) patients. Higher Knosp grade previous associated lower ( P = 0.02 0.004, respectively). On follow-up (mean, 72.5 months), impairment score improved 59.4% remained same 38.5% The axis 55.8% worsened 39.5% 10-year recurrence rate who had NTR 0% RT 63.2% without RT. Aim: To investigate the predictor of hypothalamic-pituitary-gonadal (HPG) axis recovery in male pituitary adenoma patients with trans-sphenoid extrapseudocapsular microsurgery. Methods: From June 2019 to December 2021, we retrospectively analyzed clinical data gonadal hormone changes and resection degree before after surgery nonfunctioning (NFPA) who underwent microsurgical by pseudocapsule technique same treatment group at Department Neurosurgery, Tongji Hospital. We explored predictors affecting postoperative testosterone using logistics regression analysis. Results: Among 291 adenomas, mean age was 50 years. Preoperative lower than normal 127 (43.6%); total subtotal were performed 279 (95.9%) 12 (4.1%), respectively. Postoperative follicle-stimulating (FSH) elevated 224 (77.0%) patients, luteinizing (LH) 230 (79.0%) prolactin significantly decreased 259 (89.0%) patients. Seventy-one low preoperative levels recovered levels. Univariate multivariate analysis suggested that tumor size, coagulative necrotic apoplexy (CNPA), invasiveness (P < 0.05).
