This study aimed to examine the levels, demographic differences, and associations between body image (BI) quality of life (QOL) among Japanese adolescents with congenital physical disabilities. A self-report questionnaire was administered 107 disabilities, which included variables, Multidimensional Body-Self Relations Questionnaire, version World Health Organization Quality Life Brief Version. Data were analyzed using Cronbach’s α coefficient, descriptive statistics, t-tests, one-way ANOVA, Pearson’s correlation coefficient. The mean BI score 2.96 ± 0.39. domain-specific scores as follows: health evaluation (3.49 0.67), orientation (3.11 0.57), fitness (3.03 0.93), (3.02 0.70), appearance (2.70 0.61), (2.61 0.61). QOL 3.51 0.50. social relationships (3.61 0.91), environment (3.60 0.59), (3.46 0.55), psychological (3.42 0.63). significantly varied by gender, school level, type disability, activities daily living (ADLs), while level ADLs. All domains, except (investments for enhancing appearance), positively correlated QOL. Our findings suggest that ADLs are key predictors both disabilities physical-fitness- health-related closely associated
