Existing staff assigned additional responsibilities PSSP, decreasing their motivation efficiency. While 11/18 (61%) essential drugs available PMCIs, buffer stocks maintained >50% due poor supply chain storage infrastructure. Only 6/14 (43%) investigations non-availability was shortages reagents/consumables lack sample collection−transportation system. The HMIS installed its usage sub-optimal perceived utility, few trained internet connectivity. PSSP needs address these bottlenecks as priority ensure sustainability successful scale-up.

We examine new doctors' and nurses' experiences of transitioning from training to practising as health professionals, drawing on the concept liminality. Liminality is a stage 'in-betweenness', involving uncertainty ambiguity people leave one social context reintegrate into one. Surprisingly little research has explored professionals' liminality during role career transitions, particularly in precarious resource-constrained settings. Drawing 146 qualitative interviews seven focus groups, 121 graduate medical doctors nurses through internship Kenya, we describe three aspects First, liminal professional practice, where interns realise that best practices learned nursing schools are often impossible implement resource constrained care settings; instead they learn workarounds practical norms. Second, relational identities, behind being students adopt identities responsibilities qualified professionals within pre-existing hierarchies status expertise. explain how these negotiate their status, including relation more experienced but less colleagues. also discuss cope with due disappointing inadequate supervision modelling senior colleagues then find peer support place own professions. Finally, come terms precarity working systems, abandon expectations secure, permanent employment careers, accept realities careers. all forms influence developing practices, call for further studies specific lens explore this critical period workers' inform policy practice responding global transformations healthcare professions practice.
