ABSTRACT Purpose Glucocorticoids are widely used for their anti‐inflammatory properties, but specific molecular mechanisms in treating rhegmatogenous retinal detachment with choroidal (RRDCD) remain unclear. This study aims to identify key regulatory factors the vitreous humor of RRDCD patients and analyze protein changes after hormonal intervention. Methods Vitreous fluid samples were collected during surgery from (RRD, n = 40), non‐glucocorticoid treated (nT‐RRDCD, 35), glucocorticoid‐treated (T‐RRDCD, 32). Primary outcomes reattachment status best‐corrected visual acuity (BCVA) at 6 months postoperatively. Proteomic analysis was performed using data‐independent acquisition (DIA), differentially expressed proteins validated by parallel reaction monitoring (PRM) ELISA. Results Between RRD nT‐RRDCD, 203 identified, while 295 between nT‐RRDCD T‐RRDCD. These involved complement activation, immune response, blood coagulation, MAPK signaling. Apolipoprotein D (APOD) vitronectin (VTN) positively correlated postoperative BCVA. APOD, serum amyloid A‐4 (SAA4), ubiquitin‐conjugating enzyme E2 variant emerged as potential diagnostic biomarkers RRDCD. Conclusions development involves multiple factors. mitigate damage suppressing inflammation, regulating oxidative stress, promoting cell repair. APOD VTN correlate BCVA, SAA4, show promise
