The neurotrophic system includes neurotrophins, such as brain-derived factor (BDNF) and its precursor proBDNF, which play conflicting roles in neuronal survival apoptosis, with their balance having a significant impact on neurodegenerative outcomes. While BDNF is widely acknowledged potent neurotrophin that promotes differentiation, precursor, has the opposite effect, promoting apoptosis death. This review highlights new unique aspects of BDNF/proBDNF interaction modulation apoptotic pathways disorders. It systematically discusses cross-talk signaling at molecular level, whereby activates PI3K/Akt MAPK/ERK, whereas proBDNF p75NTR sortilin to induce via JNK, RhoA, NFkB, Rac-GTPase caspase activation mitochondrial injury. Moreover, this emphasizes factors affect between levels within context neurodegeneration, including proteolytic processing, expression TrkB receptors, extrinsic gene transcription regulators. Cellular injury, stress, or pathway alterations can disrupt BDNF/proBDNF, may be involved apoptotic-related diseases like Alzheimer’s, Parkinson’s, Huntington’s diseases. provides comprehensive framework for targeting development innovative therapies managing disorders, addressing mechanistic complexity clinical feasibility interaction.
