Objectives: This study aimed to determine the positive predictive value of our NMD Suspicion Criteria in diagnosis NMDs. Other clinical factors routinely examined voice and swallowing examinations were also investigated see if they had a significant association with Methods: retrospectively medical charts patients who visited Voice Swallowing outpatient clinic between 2013 2022. Patients previously diagnosed NMDs excluded from analysis. Among remaining patients, we included those that referred neurologists for further evaluation due suspicion having an based on Criteria. The then divided into groups according status their within 2 years referral as “diagnosed”, “denied”, or “observed”. These three compared following findings; velopharyngeal insufficiency (VPI), tongue atrophy, impaired movement, dysarthria, vocal fold mobility impairment, dysphagia, involuntary gait disturbances, weight loss, sense fatigue order significantly associated Results: Of 3769 outpatients without confirmed NMDs, 37 suspected 19 (51%) VPI movement diagnostic (p = 0.014, 0.033). during speech 0.045) was more strongly than 0.076). Fatigue related factor other diseases (non-NMDs) causing problems 0.049). Conclusions: In setting, should be raised, particularly when are observed, prompting thorough assessment closure both swallowing.

Abstract Objective To investigate the effects of modified central‐part laryngectomy with pharyngeal space reduction (CPL‐PR) on patients weak deglutitive contraction, as seen in late‐stage amyotrophic lateral sclerosis (ALS). Study Design Retrospective case series. Setting Single‐institution academic center. Methods Patients ALS confined at The University Tokyo Hospital between 2019 and March 2024 whom CPL‐PR had been performed were identified. who undergone simultaneous flap surgery or no preoperative high‐resolution manofluorography done excluded. Preoperatively, penetration‐aspiration scale (PAS) scores determined via videofluoroscopic swallowing study. Functional oral intake (FOIS) manometric parameters measured compared preoperatively postoperatively. Results Eighteen identified a median age 66.5 (interquartile range [IQR]: 58.0‐74.8). PAS score was 7.5 (IQR: 5.5‐8.0), indicating severe dysphagia. There significant improvement status FOIS increasing from 1 1‐1) to 3 2‐3) months postoperatively ( P = .0002). Significant increases velopharyngeal closure integral .024) mesohypopharyngeal contractile .0001) observed. Upper esophageal sphincter (UES) resting pressure reduced .0002), UES relaxation time prolonged during < .0001). Conclusion tangible improvements bolus passage, following CPL‐PR, which contribute regaining ALS. is an option for requiring tracheostomy wish prevent aspiration regain their ability take food orally.

Vocal fold motion impairment (VFMI) is the inappropriate movement of vocal folds during respiration, leading to adduction and/or abduction problems and causing respiratory impairments. Neurodegenerative diseases (NDDs) are a wide range disorders characterized by progressive loss neurons deposition altered proteins in brain peripheral organs. VFMI may be unrecognized patients with NDDs. NDDs caused following: laryngeal muscle weakness due muscular atrophy, brainstem motor neuron degeneration amyotrophic lateral sclerosis; hyperactivity adductors Parkinson’s disease; varying degrees adductor hypertonia abductor paralysis multiple system atrophy. Management depends on whether there presence glottic insufficiency or insufficient opening with/without severe dysphagia. treatment options for from surgical interventions, including injection laryngoplasty medialization thyroplasty, behavioral therapies; opening, various available based severity underlying cause condition, continuous positive airway pressure therapy, botulinum toxin injection, tracheostomy, surgery, combination interventions. In this review, we outline mechanisms, clinical features, management provide guide physicians who encounter these features their patients. always progressive; hence, timely evaluation, proper diagnosis, appropriate patient will greatly affect vocal, respiratory, swallowing functions as well quality life.
