Inflammatory bowel disease (IBD), encompassing Crohn's and ulcerative colitis, is primarily known for its gastrointestinal manifestations. However, emerging evidence suggests a potential link between IBD an increased risk of stroke, likely mediated by chronic systemic inflammation, endothelial dysfunction, prothrombotic state. Despite this growing recognition, the exact mechanisms extent association remain unclear, highlighting critical knowledge gap. This review aims to systematically analyze exploring underlying vascular identifying factors contributing cerebrovascular events in patients. A comprehensive literature search was conducted following Preferred Reporting Items Systematic Reviews Meta-Analyses (PRISMA) guidelines across PubMed, Scopus, Google Scholar using keywords such as "IBD," "Stroke," "Chronic inflammation," "Cerebrovascular risk," "Gut-brain axis." After screening 150 studies applying inclusion exclusion criteria, six were included final synthesis. The findings suggest that inflammation plays key role increasing stroke through dysfunction heightened state, with additional atrial fibrillation during active flares further events. While biologic therapies, including tumor necrosis factor (TNF)-alpha inhibitors, are effective reducing their impact on mitigating remains inconclusive. Given independent multidisciplinary approach management crucial.
