This study compared the efficacy of apicoectomy and apical curettage with without leukocyte- platelet-rich fibrin (L-PRF) in treating large periapical lesions. Sixty-four adults (30 male, 34 female) a previously endodontically treated tooth lesion were randomized into four groups (n = 16): retrograde obturation (RG), orthograde (OG), RG + L-PRF, OG L-PRF. The development of effective biomaterials for tissue regeneration has led to the exploration blood derivatives such as leucocyte- and platelet-rich fibrin (L-PRF). A novel variant, Albumin-Enriched Platelet-Rich Fibrin (Alb-PRF), been introduced improve structural stability bioactivity, making it a promising candidate bone regeneration. This study aimed evaluate Alb-PRF’s capacity cytokine growth factor release, along with its effects on proliferation, differentiation, mineralization human osteoblasts in vitro. Alb-PRF membranes were analyzed using histological, scanning electron microscopy, fluorescence microscopy techniques. Cytokine release was quantified over seven days, osteoinductive potential evaluated MG-63 osteoblast-like cells. Structural analysis showed biphasic, highly cellularized material that releases lower levels inflammatory cytokines higher concentrations platelet-derived (PDGF) vascular endothelial (VEGF) compared L-PRF. exhibited early alkaline phosphatase activity vitro (p < 0.05) significantly increased OPG/RANKL mRNA ratio 0.05).
