Background/Objectives: Degenerative lumbar spine conditions are a major cause of disability, particularly in elderly patients or those with comorbidities. Surgical treatment often combines decompression and stabilization to address pain instability. Pedicle screws the gold standard for but pose challenges compromised bone quality. Interspinous devices have emerged as less invasive alternative, comparative studies limited. This study aimed compare clinical surgical outcomes using pedicle versus interspinous devices. Methods: A retrospective cohort was conducted on who underwent either screw fixation device at Mater Olbia Hospital between February 2020 2023. Outcomes were evaluated VAS back leg pain, SF-36 quality life, EQ-5D, SCL-90 psychological factors. Statistical analysis included paired t-tests, chi-square tests, multivariate regression. Results: total 728 included. The group consisted older higher comorbidity burdens (mean age: 68.4 vs. 59.2 years, p < 0.001). time incision size significantly shorter (p 0.001), no postoperative complications reported, compared 3.5% 0.05). Both groups demonstrated significant improvements (VAS), life (SF-36, EQ-5D), (SCL-90). Somatization paranoid ideation predictors worse group. No differences quality-of-life observed groups. Conclusions: techniques effective surgery, offering safer option high-risk patients. Psychological factors influence outcomes, underscoring need comprehensive approach addressing both physical aspects optimize patient recovery.

Background/Objectives: Lumbar disk herniation (LDH) significantly affects quality of life due to lower back and radiating leg pain. Surgical intervention, such as discectomy, is effective for symptom relief when conservative measures fail; however, psychological factors like anxiety, depression, maladaptive coping strategies may negatively impact surgical outcomes. This study aims assess the role preoperative evaluations in predicting postoperative recovery identify key functional predictors success. Methods: A prospective was conducted on 888 patients undergoing microdiscectomy LDH at Mater Olbia Hospital between December 2020 2023. Preoperative included Visual Analog Scale, Symptom Checklist 90-R, Oswestry Disability Index, Short Form 36. Logistic regression models ROC curve analysis were used significant outcomes evaluate model accuracy. Results: pain levels emotional well-being emerged strongest determinants improvement Index. The predictive demonstrated high specificity (90.2%) identifying likely benefit from surgery. Clinically improvements achieved by 69% patients, highlighting importance assessments.
