This study aimed to quantify fundus microvascular alterations in patients requiring revascularization for coronary heart disease (CHD) using swept-source optical coherence tomography angiography (SS-OCTA) and investigate the correlation between these severity of artery lesions. SS-OCTA was employed assess neurovascular parameters all participants, while Gensini score utilized gauge lesions observation group. A total 98 participants (49 CHD 49 controls) were included. Analysis revealed that vascular density (VD) superficial plexus (SVP), complex (SVC), intermediate capillary (ICP) parafoveal region, mean ICP macula, deep (DCP) (DVC) each macular region significantly reduced group compared control (P < 0.05). Multivariate logistic regression indicated lower VD values SVP, SVC, ICP, DCP DVC across regions associated with an increased likelihood severe (OR 1, P ROC curve analysis maximum area under overall macula 0.707, a cutoff value 19.64, sensitivity 65.30%, specificity 73.50%. In group, Pearson demonstrated negative (r = − 0.491, 0.001). Retinal is healthy controls. SS-OCTA-based retinal damage assessment valuable tool risk stratification early intervention CHD.
