Background: In the treatment of oncological diseases in children, search for opportunities earlier detection complications to improve results is very important. Metabolomic studies are actively conducted stratify different groups patients order identify most promising markers. Methods: Three participated this study: healthy children as a control group (n = 18), with various malignant (leukemia, lymphoma, nephroblastoma, ependymoma, etc.) 40) without complications, and 31) (inflammatory infectious). The mitochondrial metabolites (succinic fumaric acids); biomarkers related inflammation such C-reactive protein (CRP), procalcitonin (PCT), presepsin (PSP); sepsis-associated aromatic metabolites, phenyllactic (PhLA), hydroxyphenyllactic (p-HPhLA), hydroxyphenylacetic acids (p-HPhAA), were identified. Results: It was found that had profound metabolic dysfunction compared regardless presence systemic inflammatory response syndrome (SIRS) or sepsis. prognostic ability detecting sepsis high: AUROC 0.875, cut-off value (Youden index) 0.913 ng/mL, 0.774, 526 pg/mL, respectively. Conclusions: A significant increase microbial non-survivor registered already first days development indicates appropriateness assessing its timely targeted correction.
