Background Urinary tract infections (UTIs) are among the most common bacterial globally, with Enterobacteriaceae, particularly Escherichia coli and Klebsiella pneumoniae, being primary causative agents. The rising prevalence of antimicrobial resistance (AMR) in these pathogens has complicated treatment, posing a significant public health challenge. In resource-limited settings like Southwestern Uganda, limited surveillance data inappropriate use exacerbate AMR crisis. Understanding antibiotic susceptibility profiles uropathogenic Enterobacteriaceae is crucial to inform empirical treatment guide local stewardship efforts. This study investigated patterns isolated from UTI patients commonly used antibiotics. Methods A hospital-based cross-sectional was carried out Bwizibwera Health Center IV Rubaya III rural targeting 455 individuals who were confirmed have infections. These participants drawn pool 2,371 presenting urinary at two centers. Identification performed using range biochemical tests, assessed Kirby-Bauer disk diffusion technique specific analyzed summarized as frequencies percentages, results presented tables graphs. Results Out 2371 participants, (19.2%, CI 17.6 - 20.8) tested positive for Enterobacteriaceae. 455, majority females (398/455, 87.5%). mean age 41.6 (SD 19.7) years. Each had single uropathogen. Organisms highly resistant tetracycline proportion 89.5% (407/455) least meropenem 76 (16.7%) ceftriaxone 79 (17.9%). Multidrug (MDR) observed 114 (25.05%) organisms, whereas 10.77% (49/455) organisms exhibited pan-drug (PDR). addition, MDR highest isolates females, PDR 60 years above. Conclusion Our revealed high uropathogens South Western Uganda predominantly affecting women. showed varying degrees (MDR, extensive drug (XDR), PDR), including carbapenem-resistant strains E. coli, Klebsiella, Enterobacter spp., threat. findings highlight urgent need revised prescription practices, strengthened stewardship, continuous uropathogen effective strategies.
