Human errors (HEs) are common problems in manual assembly processes, impacting product quality and resulting additional costs. Based on expert judgments, this study aims to identify the most significant factors affecting HEs processes explore cause-and-effect relationships among those factors. In order achieve objective, a proposed model is constructed using two types of Multi-Criteria Decision-Making (MCDM) techniques. Firstly, rounds fuzzy Delphi method (FDM), twenty-seven with an influence score 0.7 or higher were found have major impact during at least 75% consensus experts. After that, given experts third round analyze decision-making trial evaluation laboratory (DEMATEL) method. MCDM techniques, symmetry refers important property that can be used find between variables. It based principle relative importance preference variables should remain same regardless their positions roles. Therefore, factor approaches take into account ensure accurately represented, leading more reliable outcomes. The reliability normality surveying data examined SPSS 22.0 software program. results revealed training level, poor workplace layout, lack necessary tools, experience as root causes. Moreover, failure address error-causing problem, unintentional unsafe acts, fatigue, error visual perception effect (dependent) findings help organizations make better-informed decisions how reduce worker interest contribute provide good basis for reaching final assembled parts.

Functional safety is an emerging trend for design in the automobile industry. This study innovative exploration of a hazard analysis and quantitative risk assessment (QRA) methodology based on functional vehicular fuel cell systems. In this study, potential hazards that vehicles may encounter during operation were identified using generic list. The failure modes influences main components system determined via operability studies. Furthermore, risks consequences explored QRA method. frequency unavailability fuel-cell calculated, architecture was optimized redundant with focus hydrogen safety. results showed could effectively reduce leakage, leakage reduced by 94.4 % 36.8 %, respectively. Thus, provides powerful insights future research improvement
