A 3-yr-old, male Komodo dragon (Varanus komodoensis) presented acutely in early October 2021 with an abnormal posture and asymmetric tetraparesis. The subadult lizard had spent the previous 4 months either outdoor space or indoor open windows. Physical examination, radiographs, complete blood count chemistry panel failed to reveal cause of neurologic signs. West Nile virus (WNV) plaque reduction serum neutralization on presentation was negative (1:20), as WNV polymerase chain reaction (PCR) testing viral isolation whole blood. Empirical treatment a daily corticosteroid, course antibiotics, encouragement mild exercise initiated. Magnetic resonance imaging brain spinal cord unremarkable, lumbar cerebrospinal fluid (CSF) tap revealed mononuclear pleocytosis protein elevation. Follow-up serology rising titer seroconversion 13 days after (1:80) 32 (1:640); however, PCR CSF negative. Thi gradually improved over six weeks before being put back public display 74 presentation. remained high (1:640) through winter 2021-22, anamnestic response observed following vaccination series killed equine vaccine spring 2022. There is brief report infection captive crocodile monitor salvadorii), unpublished case same species at first author's institution. This disease from presumptive dragon. We recommend that be differential diagnosis if varanid present acute signs history arthropod vector exposure, particularly summer fall.
