Background/Objectives: While the concept of viral community-acquired pneumonia (CAP) changed with COVID-19, role non-influenza viruses as a cause CAP is less clear. The aim this study was to describe clinical course, risk factors, inflammatory profiles, antibiotic use, outcomes and complications adenoviral (AdV) CAP. Methods: A prospective, non-interventional, observational cohort included consecutively hospitalized immunocompetent adult patients AdV during an 18-month period. Clinical laboratory data, including lymphocyte subpopulations serum cytokine profiles were collected correlated outcomes. Results: Fifty-eight included; 81% males, median age 33 (IQR 28-41) years 62% without any comorbidities. All initially had high-grade fever for duration 6 (5-7) days respiratory symptoms. Increased CRP procalcitonin, lymphopenia, mild thrombocytopenia liver injury frequent. Radiographic findings mimicked bacterial (83% unilateral involvement). Twenty-two (38%) criteria severe CAP, these higher NLR, AST, ALT, LDH CK, lower T-lymphocyte CD4+ count. In comparison influenza IL-2, IL-1β, IL-8, IL-10, CXCL10 MCP-1, TGF-β1 concentration. Thirteen required low-flow oxygen therapy, 13 advanced support. Complications occurred in 29%, one fatal outcome. all received empirical after detection it stopped 21%, although only patient detected possible coinfection. Conclusions: Since clinically radiologically indistinguishable from associated prolonged course lack response antibiotics. This emphasizes importance testing which could lead more rational antimicrobial treatment.
