BACKGROUND Hydatid cyst disease, caused by Echinococcus granulosus , primarily affects the liver and lungs, but it can also develop in rare locations such as kidneys, thyroid, subcutaneous tissues, bones, mediastinum. These atypical presentations often pose diagnostic challenges, they mimic benign malignant pathologies, leading to potential misdiagnoses inappropriate treatments. Early accurate detection of hydatid cysts uncommon sites is crucial for optimal patient management. CASE SUMMARY This case report series presents five patients with located anatomical regions: The kidney, lumbar tissue, gluteal soft posterior mediastinum, thyroid gland. exhibited diverse clinical symptoms, including hematuria, palpable masses, localized pain, chronic cough. Diagnosis was confirmed through a combination imaging techniques-ultrasound, computed tomography, magnetic resonance imaging-along serological testing. All cases were managed antiparasitic therapy (albendazole), selected cases, surgical excision performed prevent complications rupture or secondary infections. Post-treatment follow-up demonstrated complete resolution stable cystic lesions, no signs recurrence. CONCLUSION Recognizing essential avoid misdiagnosis ensure appropriate treatment strategies. Radiological plays key role distinguishing from other neoplastic conditions, while tests aid confirmation, particularly endemic regions. A multidisciplinary approach, integrating radiology, evaluation, expertise, critical effective diagnosis highlights need increased awareness extrapulmonary extravisceral emphasizing its significance differential practice.
