ABSTRACT Background The coexistence of type two diabetes (T2D) and cardiovascular diseases (CVD) in patients with overweight increases the risk macro-vascular complications. Some studies are controversial results Ramadan fasting (RF) T2D+CVD. Objective To compare different RF (antique modern) on anthropometric, glycemic, blood pressure (BP), lipids, heel bone mineral density (HBMD), ejection fraction (EF) Methods An open, 60-day, controlled, single-center, randomized clinical trial included 51 (29 women) T2D+CVD: 26 Main RF); 25 Controls (modern RF). Primary endpoints: weight loss, glucose and, insulin, BP. Secondary HBMD, EF. Results Patients lost –8.02 kg ( P <0.0001), –2.67 <0.025); BMI significantly decreased but did not decrease P> 0.025). During a 30-day follow-up, regain WC; regained WC. In BP, all glycemic parameters (fasting glucose, immunoassay HOMA-IR), lipids (cholesterol, triglyceride, HDL), EF without heart failure (HF) were normalized at <0.0001).
