Aneurysms and dissections represent some of the most serious cardiovascular diseases. The prevailing theory posits that mechanical overloading vessel wall is underlying cause. Inspired by Barkhordarian et al , authors present matrix metalloproteinases (MMPs) their inhibitors in immunohistological analyses as contributing factors pathophysiology aortic aneurysms (AA). Data analysis MMP-1, MMP-9, tissue (TIMPs), including TIMP-1 TIMP-2 expression reveals a varied distribution between adventitia media non-uniform investigated markers. These elements, key components extracellular (ECM), indicate formation AA not solely driven endoluminal pressure loading wall. Instead, degenerative processes within ECM elements contribute significantly. Importantly, do necessarily imply dissection. Tissue destruction, allowing blood flow entry, arises from reduced oxygen supply to media, primarily due incomplete capillarization or neocapillarization.

The pathobiology of aortic disease is linked to region: atherosclerosis for abdominal aorta, primary medial degeneration or aortitis ascending thoracic and all causes descending aorta thoracoabdominal lesions. pathogenesis dissection involves damage the outer media from impaired perfusion dysfunctional vasa vasorum, formation discrete foci disrupted vascular smooth muscle cell-elastic fiber extension-contractile units, imbalance radial sheer stress across wall, thereby creating an intimal tear linear dissection. Thoracic aneurysms develop chronic progression coupled with weakening remodeled adventitia, allowing dilatation. Precipitating factors include hypertension mutations genes regulating units. Criteria are presented distinguishing genetic acquired dissections, important implications therapeutic surgical decisions in care these patients.

An aneurysm is defined as a dilation of the arterial wall with diameter exceeding 1.5 times normal vessel concerned. Aortic aneurysms (AAs) can develop at any level but are mostly found abdominal and infrarenal levels extend to iliac arteries. AAs usually asymptomatic most often discovered incidentally during various imaging investigations for other conditions. Rupture an AA dramatic, being one causes sudden cardiac death. Surgical treatment and, more recently, endovascular only effective methods repair. In this study, we screened diagnosis in patients stable exertional angina who had indications coronary angiography. The study was carried out period 2021–2023 Institute Cardiovascular Diseases Timişoara, Romania. Of 2458 required angiography, number 1844 (75%) least stenotic atheromatous plaque, these 312 AAs, which 173 aorta.
