Objective: About 10-15% of sub-fertility cases facing the clinicians in reproductive clinics remains unexplained. Opinions regarding optimal treatment plan for them exhibit a wide differences among fertility specialists. This study aims to assess outcome ICSI and predictors success couples with unexplained sub-fertility. Methods: Sixty six sub-fertile infertility were included. was done all. They divided into 2 groups pregnant non-pregnant. followed up retrospectively evaluate results compared between them. Results: Chemical pregnancy rate 42.2%. Pregnant females significantly : younger mean age 26.7±4.2 vs 28.6±3.6, lower BMI 27.6± 4.9 30.2± 4.9, cycle day LH level 2.3 ±0.99 2.9±0.94 had slightly thinner endometirum 3.3± 0.68 3.9 ±1.1 than non-pregnant ones. Females both exhibited no significant difference response controlled ovarian stimulation, total number retrieved mature oocytes comparable, only exception is immature which 0.85± 1.1 fertilization rate, embryos, cleavage embryos’ quality. Conclusion: Intra cytoplasmic sperm injection has ability, within certain limits, bypass obstacles natural failure should be considered those as early possible before female partner get older.
