Necrotizing pancreatitis (NP) is a severe complication of acute pancreatitis, characterized by necrosis pancreatic and peripancreatic tissues, leading to significant morbidity mortality. The role necrosectomy, the surgical removal necrotic tissue, in management NP has evolved over past few decades, moving from early aggressive intervention more conservative stepwise approach. This narrative review explores historical perspectives, current practices, future trends necrosectomy NP. Early studies favored open debridement; however, high mortality rates associated with prompted shift towards minimally invasive techniques, delayed interventions, "step-up approach," combining percutaneous drainage surgery.
