Abstract Older adults are priority coronavirus disease 2019 (COVID-19) vaccine recipients. Our aim was to investigate the prevalence of and factors influencing hesitancy in older living care homes communities. A self-administered COVID-19 survey conducted from September 2021 December among people aged ≥60 years eight (382 participants) community (112 Taizhou, China. We found that 58.1% 36.6% those were hesitant receive there a statistically significant difference (P<0.001). Multiple logistic regression results indicated main hesitation being male (OR=1.67, 95% CI: 1.01-2.76); their cognitive level, including having high perceived risk infection (OR=3.06, 1.73-5.43) or perception low safety (OR=3.08, 1.545- 6.145); anxiety (OR=3.43, 1.96-5.99); no previous influenza vaccination (OR=1.82, 1.13-2.93); whereas for comorbid chronic diseases (OR=3.13, 1.11- 8.78) workers not recommending (OR=8.223, 1.77-38.27). The proportion who significantly higher than community. Targeted measures should be implemented reduce improve rates response special environment characteristics this population.
