Abstract Posttraumatic stress disorder (PTSD) is a chronic and disabling psychiatric prevalent in military veterans. Epigenetic mechanisms have been implicated the etiology of PTSD, with DNA methylation being most studied to identify novel molecular biomarkers associated this disorder. We performed one largest single-sample epigenome-wide association studies (EWAS) PTSD date. Our sample included 1135 male European–American U.S. veterans who participated National Health Resilience Veterans Study (NHRVS). was collected from saliva samples Illumina HumanMethylation EPIC BeadChip used for analysis. assessed using Checklist. An EWAS conducted linear regression adjusted age, cell-type proportions, first 10 principal components, smoking status. After Bonferroni correction, we identified six genome-wide significant (GWS) CpG sites past-month three CpGs lifetime ( p range = −10 –10 −8 ). These map genes involved immune function, transcription regulation, axonal guidance, cell signaling, protein binding. Among these, SENP7 , which regulation has linked risk-taking behavior alcohol consumption studies, replicated an independent veteran cohort downregulated medial orbitofrontal cortex postmortem brain tissue. findings suggest potential epigenetic that may help inform pathophysiology other trauma-affected populations.

Posttraumatic stress disorder (PTSD) is a serious mental health that can be effectively treated with empirically based practices. PTSD screening essential for identifying undetected cases and providing patients appropriate care.To determine whether the Primary Care screen Diagnostic Statistical Manual of Mental Disorders (Fifth Edition) (PC-PTSD-5) diagnostically accurate acceptable measure use in Veterans Affairs (VA) primary care clinics. This cross-sectional, diagnostic accuracy study enrolled participants from May 19, 2017, to September 26, 2018. Participants were recruited clinics across 2 VA Medical Centers. Session 1 was conducted person, session completed within 30 days via telephone. A consecutive sample 1594 veterans, aged 18 years or older, who scheduled visit recruited. Data analysis performed March 2019 August 2020.In 1, battery questionnaires. In 2, research assistant administered PC-PTSD-5 participants, then clinician assessor blind results structured interview PTSD.The range cut points overall gender assessed, performance evaluated by calculating weighted κ values. In total, 495 veterans (31%) participated, 396 all measures included analyses. demographically similar population (mean [SD] age, 61.4 [15.5] years; age range, 21-93 years) predominantly male (333 [84.1%]) White (296 394 [75.1%]). The had high levels (area under receiver operating characteristic curve [AUC], 0.927; 95% CI, 0.896-0.959), men (AUC, 0.932; 0.894-0.969), women 0.899, 0.824-0.974). point 4 ideally balanced false negatives positives men. Background: Antonovsky's concept of sense coherence (SOC) – as a global orientation reflecting an individual's feeling confidence in both the predictability their internal and external environment ability to cope with stressful challenging situations life shows negative association symptoms post-traumatic stress. However, single studies varying study characteristics provide heterogeneous effect size estimations. Objective: The purpose current is investigate relationship between SOC stress disorder (PTSD) symptom severity for first time on meta-analytical level.Method: random-effects meta-analysis based zero-order correlations (r) consists 47 independent samples out 45 (N = 10,883).Results: After correcting sampling error, mean correlation PTSD was M(r) −.41 (excluding four outliers: −.39). this could not be generalized all types owing substantial remaining heterogeneity. Subsequent moderator analyses investigating influence different measures, trauma type duration, age gender imbalances per sample did reveal significant moderating effects. Conclusions: reveals severity: higher levels are associated lower severity. Thus, future research should progress question whether causal, by which factors it moderated.Abbreviations: CD, Cook's distance; DSM, Diagnostic Statistical Manual Mental Disorders; (G)RR, general (and specific) resistance resources; IES(-R), Impact Event Scale (Revised); PDS, Posttraumatic Stress Scale; PRISMA, Preferred Reporting Items Systematic Reviews Meta-Analyses; PTSD, disorder; SDR, standard deleted residual; SOC, coherence; SOC-R, Sense Coherence Revised

Objective This study examined clinical and retention outcomes following variable length prolonged exposure (PE) for posttraumatic stress disorder (PTSD) delivered by one of three treatment modalities (i.e., home-based telehealth [HBT], office-based [OBT], or in-home-in-person [IHIP]). Method A randomized trial design was used to compare variable-length PE through HBT, OBT, IHIP. Treatment duration number sessions) determined either achievement a criterion score on the PTSD Checklist Diagnostic Statistical Manual-5 (DSM-5; DSM-5) two consecutive sessions completion 15 sessions. Participants received via HBT (n = 58), OBT 59) IHIP 58). Data were collected between 2012 2018, diagnosed using Clinician-Administered Scale DSM-5 (CAPS-5), administered at baseline, posttreatment, 6 months completion. The primary outcome CAPS-5 severity. Secondary included self-reported depression symptoms, as well dropout. Results effectiveness did not differ modality across any time point; however, there significant difference in Veterans (odds ratio [OR] 2.67; 95% confidence interval [CI] 1.10, 6.52; p .031) (OR 5.08; CI 2.10; 12.26; < .001) conditions significantly more likely than veterans drop out treatment. Conclusions Providers can effectively deliver in-home, in-person although rate higher care.
