Importance The effects of probiotic interventions on colonization with resistant bacteria and early microbiome development in preterm infants remain to be clarified. Objective To examine the efficacy Bifidobacterium longum subsp infantis , animalis lactis (BB-12), Lactobacillus acidophilus (La-5) probiotics prevent multidrug-resistant organisms or highly epidemic (MDRO+) shape toward eubiotic state healthy full-term infants. Design, Setting, Participants multicenter, double-blinded, placebo-controlled, group sequential, phase 3 Priming Immunity at Beginning Life (PRIMAL) randomized clinical trial, conducted from April 2018 June 2020, included gestational age 28 32 weeks 18 German neonatal units. Donor human milk (DHM) provides myriad nutritional and immunological benefits for preterm low birthweight infants. However, pasteurization leaves DHM devoid of potentially beneficial microbiota. In the present study, we performed microbiome transplantation from freshly collected mother's own (MOM) into pasteurized DHM. Small volumes MOM (5%, 10%, or 30% v/v) were inoculated incubated at 37 °C up to 8 h.
