Lung cancer (LC) is one of the most common worldwide. Tumor-associated macrophages (TAMs) are important component tumor microenvironment (TME) and closely related to stages occurrence, development, metastasis. Macrophages plastic can differentiate into different phenotypes functions under influence signaling pathways in TME. The classically activated (M1-like) alternatively (M2-like) represent two polarization states macrophages. M1 exhibit anti-tumor functions, while M2 considered support cell survival Macrophage involves complex pathways, blocking or regulating these enhance macrophages' effects has become a research hotspot recent years. At same time, there have been new discoveries regarding modulation TAMs towards an phenotype by synthetic natural drug components. Nanotechnology better achieve combination therapy targeted delivery drugs, maximizing efficacy drugs minimizing side effects. Up now, nanomedicines targeting various active substances for reprogramming made significant progress. In this review, we primarily provided comprehensive overview crosstalk between cells LC microenvironment. The fight against lung adenocarcinoma (LUAD) is challenged by tumor microenvironment (TME)-mediated drug resistance, which limits effective treatment. This study examines the LUAD TME and identifies four distinct subtypes through multi-omics profiling: immune-rich, immune-exhausted, stromal-dominant, TME-desert. Each subtype has unique molecular features, diversity, links to clinical outcomes. Immune-rich respond better immune checkpoint inhibitors, while stromal-dominant TME-desert show resistance treatment poor prognosis. Molecular analysis uncovers subtype-specific mutations, chromosomal instability, altered signaling pathways, pointing potential therapeutic targets. In silico screening promising treatments for resistant subtypes. These findings, validated in independent cohorts, highlight critical role of response, providing insights personalized strategies LUAD.
