Abstract Osteoporosis is a widely observed condition characterized by the systemic deterioration of bone mass and microarchitecture, which increases patient susceptibility to fragile fractures. The intricate mechanisms governing homeostasis are substantially impacted extracellular vesicles (EVs), play crucial roles in both pathological physiological contexts. EVs derived from various sources exert distinct effects on osteoporosis. Specifically, released osteoblasts, endothelial cells, myocytes, mesenchymal stem cells contribute formation due their unique cargo proteins, miRNAs, cytokines. Conversely, secreted osteoclasts immune promote resorption inhibit formation. Furthermore, use as therapeutic modalities or biomaterials for diagnosing managing osteoporosis promising. Here, we review current understanding impact homeostasis, including classification biogenesis regulatory present an overview latest research progress treating using EVs. Finally, discuss challenges prospects translational

The term “peri-implantitis” (peri-implantitis) refers to an inflammatory lesion of the mucosa surrounding endosseous implant and a progressive loss peri-implant bone that supports implant. Recently, it has been suggested increased sensitivity implants infection quick elimination supporting tissue after may be caused by dysregulated mucosal immune response. Macrophages are polarized in response environmental signals play multiple roles peri-implantitis. In peri-implantitis samples, recent investigations have discovered considerable increase M1 type macrophages, with macrophages contributing pro-inflammatory brought on bacteria, whereas M2 contribute inflammation remission repair. effort better understand pathogenesis suggest potential immunomodulatory treatments for direction macrophage polarization patterns, this review summarizes research findings related compares them periodontitis.

Abstract The oral and maxillofacial region is a highly complex area composed of multiple tissue types bears various critical functions the human body. Diseases in this pose significant diagnostic management challenges; therefore, exploring new strategies for early diagnosis, targeted treatment, reconstruction key to improving patient prognosis quality life. Extracellular vesicles are group heterogeneous lipid-bilayer membrane structures secreted by most cell types, including exosomes, microvesicles, apoptotic bodies. Present body fluids tissues, they act as messengers via transfer nucleic acids, proteins, metabolites recipient cells. To date, studies have revealed different roles extracellular physiological or pathological processes, well applications disease prognosis, treatment. importance specificity dental tissues indicate that derived from promising further research. This paper reviews published data on cells, fluids, regions, summarizes latest advances extensive sources, concludes with focus current research progress application prospects engineered exosomes science.

Abstract Exosomes are the smallest subset of extracellular signaling vesicles secreted by most cells with ability to communicate other tissues and cell types over long distances. Their use in regenerative medicine has gained tremendous momentum recently due their be utilized as therapeutic options for a wide array various diseases. Over 5000 publications currently being published on this topic yearly, many which dental space. This extensive review article is first scoping aimed at summarizing all uses exosomes dentistry. A total 944 articles were identified using field either regenerative/therapeutic potential or diagnostic purposes derived from oral cavity. In total, 113 research selected (102 vitro, 60 vivo, 50 studies included both). Therapeutic commonly pulps, periodontal ligament cells, gingival fibroblasts, stem exfoliated deciduous teeth, apical papilla have been shown facilitate number including bone, cementum, ligament, nerves, aid orthodontic tooth movement, relieve temporomandibular joint disorders, among others. Results demonstrate that led positive outcomes 100% studies. bone field, found perform equally well better than rhBMP2 while significantly reducing inflammation. Periodontitis animal models treated simple injections benefits even observed when administered intravenously. much more stable growth factors far resistant against degradation pathogens routinely periodontitis environment. Periodontitis is a chronic inflammatory disease mediated by dysbiosis of the oral microflora, resulting in destruction periodontal tissue. Increasing evidence suggested that mesenchymal stem cell (MSCs) and exosomes derived from MSCs play critical role tissue regeneration. However, whether cells exfoliated deciduous teeth (SHED)-secreted can improve therapeutic potential periodontitis largely unknown.

Abstract Extracellular vesicles (EVs) are cell-derived with a phospholipid bilayer measuring 50–150 nm in diameter demonstrated therapeutic potentials. Limitations such as the natural biodistribution (mainly concentrated liver and spleen) short plasma half-life of EVs present significant challenges to their clinical translation. In recent years, growing research indicated that engineered enhanced targeting lesion sites have markedly promoted efficacy. However, there is dearth systematic knowledge on advances engineering for targeted delivery. Herein, we provide an overview mechanisms, techniques, translations applications. Enrichment at may be achieved through recognition tissue markers, pathological changes, circumvention mononuclear phagocyte system (MPS). Alternatively, external stimuli, including magnetic fields ultrasound, also employed. EV techniques fulfill functions includes genetic engineering, membrane fusion, chemical modification physical modification. A comparative statistical analysis was conducted elucidate discrepancies between diverse size, morphology, stability, efficacy vitro vivo. Additionally, summary registered trials utilizing from 2010 2023 has been provided, full discussion perspectives. At present, extracellular vesicles (EVs) are considered key candidates for cell-free therapies, including treatment of allergic and autoimmune diseases. However, their therapeutic effectiveness, dependent on proper targeting to the desired cells, is significantly limited due reduced bioavailability resulting from rapid clearance by cells mononuclear phagocyte system (MPS). Thus, developing strategies avoid EV elimination essential when applying them in clinical practice. On other hand, malfunctioning MPS contributes various immune-related pathologies. Therapeutic reversal these effects with EVs would be beneficial could achieved, example, modulating macrophage phenotype or regulating antigen presentation dendritic cells. Additionally, intended macrophages replication repackaging molecules into new vesicle subtype can allow specific appropriate populations acceptor Herein, we briefly discuss under-explored aspects MPS-EV interactions that undoubtedly require further research order accelerate use EVs.
