Clinical development of BTK kinase inhibitors for treating autoimmune diseases has lagged behind these drugs cancers, due in part from concerns over the lack selectivity and associated toxicity profiles first generation drug candidates when used long term treatment immune mediated diseases. Second have made great strides limiting off-target activities distantly related kinases, though they had variable success at cross-reactivity within more closely TEC family kinases. We investigated specificity profiles, properties, disease signaling pathways, clinical indications, trial successes failures 13 inhibitor tested phase 2 or higher trials representing 7 inflammatory immune-mediated focused on rheumatoid arthritis (RA), multiple sclerosis (MS), systemic lupus erythematosus (SLE) where majority nonclinical studies been reported, with additional information pemphigus vulgaris (PV), Sjogren’s (SJ), chronic spontaneous urticaria (CSU), graft versus host (GVHD), asthma included available. While improved kinases outside less profile distinction was evident family. Analysis genetic associations RA, MS, SLE biomarkers members revealed that may not be drivers disease. They are, however, mediators pathways pathophysiology particular B cell myeloid differentiation as well autoantibody implicated Successes clinic PV, ITP, GVHD, but SJ support concept plays an important role mediating pathogenic processes amenable to therapeutic intervention, depending Based data collected this study, we propose current compound characteristics achieved selectivity, safety, coverage requirements necessary deliver benefit.

Systemic rheumatic diseases (SRDs) are chronic, inflammatory, autoimmune disorders with the presence of autoantibodies that may affect any organ or system. Liver dysfunction in SRDs can be associated prescribed drugs, viral hepatitis, alternative hepatic comorbidities and coexisting liver (AILDs), requiring an exclusion secondary conditions before considering involvement. The patterns overlap depend predominantly on genetic determinants common susceptible loci widely distributing both disorders. In AILDs, it is important to identify overlapping at early stage since such a coexistence influence disease course prognosis. Commonly co-occurring AILDs Sjögren syndrome (SS), rheumatoid arthritis (RA) systemic lupus erythematosus (SLE) hepatitis (AIH), SS, RA sclerosis primary biliary cholangitis. Abstract Sjögren’s disease is a heterogeneous autoimmune disorder that may be associated with systemic manifestations such as pulmonary or articular involvement. Systemic complications have prognostic implications and need to identified managed in timely manner. Treatment should tailored the type severity of organ involvement, ideally based on multidisciplinary evaluation.

Abstract Sjögren's syndrome‐related dry eye (SSDE) is a severe subtype characterized by significant immune cell attacks on the lacrimal gland. However, delivering immunosuppressive drugs to glands for SSDE therapy safely and sustainably poses challenges in clinical practice. Herein, ROS‐responsive microneedle patch with detachable functionality (CE‐MN) developed enable straightforward minimally invasive administration gland area penetrating periocular skin. CE‐MN loaded cyclosporin A anti‐inflammatory drug epigallocatechin gallate, latter also serving as cross‐linker matrix. Poly(N‐isopropylacrylamide‐co‐butylacrylate), temperature‐sensitive polymer utilized fabricate separable layers that allow controlled detachment of base from needle, reducing patient discomfort. capable modulating release responding ROS, facilitating on‐demand release, accumulation Compared traditional drops, facilitated long‐acting delivery more than 48 h, demonstrating potent effects an mouse model scavenging ROS inhibiting proliferation Th1, Th17 cells, macrophages. Overall, this potentially offers novel approach treating other ocular chronic diseases.

An aberrant immune response cause myelin destruction that covers axons in brain, spinal cord, optic nerves. Systemic lupus erythematosus an autoimmune characterized by alteration B cell activation, while Sjögren’s syndrome heterogeneous altered responses. The etiology all these diseases very complex, including interrelationship between genetic factors, principally associated genes, environmental factors such as infectious agents. Sjögren's syndrome (SS) is a systemic autoimmune disease, characterized by lymphocytic infiltration of the secretory glands. This leads to dryness main mucosal surfaces such as mouth, eyes, nose, larynx, pharynx, and vagina. Although there little morbidity data at initial diagnosis, SS may be serious with extra mortality caused hematological cancer. The cause unknown, but factors postulated play role include genetic environmental factors, hormonal abnormality, viral infection. Under influence these immune system becomes abnormal tissue damaged. In this study, we summarize recent developments in our understanding relationship between infections, including Epstein–Barr virus (EBV), hepatitis C (HCV), human T cell lymphotropic type 1 (HTLV-1), cytomegalovirus (CMV), immunodeficiency (HIV).
