Defocus Incorporated Soft Contact (DISC) lenses with +2.50 D myopic defocus reduced myopia progression by 25% in a previous randomised clinical trial (RCT). The current study aimed to evaluate if stronger defocus, +3.50 variable (DISC3.5plus), could slow compared single vision (SV) soft contact 12-month RCT. Conducted from December 2018 January 2021, the RCT randomly assigned children wear DISC3.5plus (n = 87) or SV 80) lenses. Myopia and axial elongation were between two groups. Abstract In this paper, three key questions regarding our understanding of myopia are addressed: predictors onset, the mechanism underlying its development and potential treatments to slow progression. To explore these aspects, first, a non‐invasive biomarker using chicken model was investigated. A strong correlation found between both increased ultraviolet (UV) fundus reflectivity thinning retinal nerve fibre layer (RNFL). Through electron microscopy, it observed that, within hours vision deprivation, significant myelin sheaths around large axons in RNFL occurred, accompanied by an increase UV reflectivity—changes that took place even before eye elongation. Purpose: To investigate the association of near work, time outdoors, and sleep duration with myopic regression 5 years after small incision lenticule extraction (SMILE) femtosecond laser–assisted in situ keratomileusis (FS-LASIK) . Methods: This cross-sectional study included patients who received SMILE or FS-LASIK at Beijing Tongren Hospital ago. The underwent ophthalmic examinations including visual acuity, intraocular pressure, subjective refraction, slit-lamp examination, keratometry, corneal topography, optical coherence tomography, fundus examination from January 2020 to March 2023. Fluorescein break-up was measured Ocular Surface Disease Index questionnaire completed exclude dry eye. A self-administered used collect data on work exposure, physical activities, duration. Results: total 323 eyes were analysis, a 5-year incidence rate FSLASIK 16.1%. After adjusted for all confounders, more than 8 hours/day revealed significant (odds ratio: 2.461; 95% CI: 1.143 5.298, P = .021), particularly younger adults, women, high myopia treatment. between restricted women (near significant). However, no associations observed continuous outdoors regression. Conclusions: Excessive exposure associated greater FS-LASIK.
