Abstract Mendelian randomization (MR) is a genetic epidemiological technique that uses variation to infer causal relationships between modifiable exposures and outcome variables. Conventional observational studies are subject bias from range of sources; MR analyses can offer an advantage in they less prone as use variants inherited at conception “instrumental variables”, which proxies exposure. Diabetic neuropathy (DN) is a common complication of type 2 diabetes mellitus (T2DM), yet its impact on bone mineral density (BMD) remains unclear. This study combines cross-sectional and meta-analytic methods to comprehensively explore the relationship between DN osteoporosis in T2DM patients. The included 523 patients, classified into non-DN groups. BMD was measured using dual-energy X-ray absorptiometry (DXA), multivariate analysis identified osteoporosis-related factors. meta-analysis studies published from 1999 2021, comparing patients with without DN. results showed that while significant differences clinical characteristics biochemical indices (such as age, HbA1c, 25(OH) vitamin D levels) were observed groups, no association found after adjusting for covariates like body mass index blood pressure. Background While Sodium-glucose cotransporter 2 (SGLT2) inhibitors are effective in managing diabetes and reducing cardiovascular risk, concerns about their association with lower limb complications, including, osteomyelitis, ulcers, peripheral artery disease (PAD), persist. This study employs Mendelian Randomization (MR) to assess the causal relationship between SGLT2 these safety outcomes. Methods A two-sample drug-target MR approach was used, complemented by a one-sample genetic analysis. Six SNPs were selected as instrumental variables proxy effect of inhibition. Primary outcomes major outcomes, including PAD, cellulitis. The primary analytical method generalized inverse variance-weighted (IVW) approach, along several sensitivity analyses.
