Self-collection of saliva samples has attracted considerable attention in recent years, particularly during the coronavirus disease 2019 pandemic. However, studies investigating detection other common respiratory pathogens are limited. In this study, nasopharyngeal swabs (NPS), oropharyngeal (OPS), and "hock-a-loogie" (HLS) were collected from 469 patients to detect 13 pathogens. To determine the clinical indicators predictive of refractory Mycoplasma pneumoniae pneumonia (RMPP) in children and develop a robust model to aid early identification management. A retrospective cohort study was conducted on 338 diagnosed with RMPP out total 1500 cases at single tertiary hospital from May 2021 November 2023. Clinical demographic data analyzed included age, gender, parents’ educational level, household income, body mass index, allergic constitution, laboratory findings such as white blood cell count, neutrophil lymphocyte counts, platelet levels C-reactive protein (CRP), D-dimer, procalcitonin. Univariate multivariate logistic regression analyses were performed identify significant predictors RMPP, developed. Among cohort, 52.4% female, mean age 6.07 ± 2.78 years. Multivariate analysis identified several poor prognosis, including higher longer duration fever, elevated levels, increased ratio ratio. The demonstrated outstanding diagnostic performance, an area under receiver operating characteristic curve 0.963 (95% confidence interval: 0.946–0.981).
