Abstract Background Major depressive disorder (MDD) is a leading cause of disability and premature mortality. This study compared the overall survival (OS) between patients with MDD non-MDD controls stratified by gender, age, comorbidities. Methods nationwide population-based cohort utilized longitudinal patient data (01/01/2010 – 12/31/2020) from Hungarian National Health Insurance Fund database, which contains healthcare service for population. Patients were selected matched 1:1 to those without using exact matching. The rates conversion bipolar (BD) or schizophrenia also investigated. Results Overall, 471,773 included in each groups. had significantly worse OS than (hazard ratio [HR] = 1.50; 95% CI: 1.48−1.51; males HR 1.69, 1.66–1.72; females 1.40, 1.38–1.42). estimated life expectancy was 7.8 6.0 years less that aged 20 45 years, respectively. Adjusted analyses based on presence baseline comorbidities showed (adjusted 1.29, 1.28–1.31). After 11 follow-up, cumulative conversions BD 6.8 3.4%, Converted non-converted patients. Conclusions Compared controls, higher mortality rate MDD, especially and/or who have converted schizophrenia, suggests early detection personalized treatment may reduce diagnosed MDD.
