Ischemic stroke was a major cause of mortality and disability worldwide. Mechanical thrombectomy (MT) has improved acute ischemic treatment by restoring blood flow in large vessel occlusions. Yet, reperfusion injury remains challenge, necessitating adjunctive neuroprotective strategies to enhance recovery. N-butylphthalide (NBP), with its anti-inflammatory antioxidative properties, may improve functional outcomes post-MT. This retrospective study analyzed 120 patients treated MT at single institution from December 2020 2022. Patients were divided into routine care group (n = 56) an NBP 64). Baseline characteristics, comorbidities, biochemical profiles assessed. Functional measured the modified Rankin Scale (mRS) 90 days.
