Major depressive disorder (MDD) is a common and complex mental disorder, that adversely impacts an individual’s quality of life, but its diagnosis treatment are not accurately executed symptom-based approach utilized in most cases, due to the lack precise knowledge regarding pathophysiology. So far, first-line treatments still based on monoamine neurotransmitters. Even though there lot progress this field, mechanisms seem get more confusing, also getting controversial. In study, we try review broad advances neurotransmitters field MDD, update effects many advanced neuroscience studies. We propose hypothesis paid special attention their new pathways for such as inflammation, oxidative stress, neurotrophins, neurogenesis, especially glial cells, which have recently been found play important role neurodegenerative disorders, including MDD. addition, will extend basic emotions; suggested our previous reports, three different roles emotions: dopamine—joy, norepinephrine—fear (anger), serotonins—disgust (sadness). A good nutritional status is important for maintaining normal body function and preventing or mitigating the dysfunction induced by internal external factors. Nutritional deficiencies often result in impaired function, and, conversely, intakes at recommended levels can resume further enhance functions. Abstract Major depressive disorder (MDD) is the most prevalent psychiatric worldwide and severely limits psychosocial function quality of life, but no effective medication currently available. Circular RNAs (circRNAs) have been revealed to participate in MDD pathological process. Targeted delivery circRNAs without blood‐brain barrier (BBB) restriction for remission represents a promising approach antidepressant therapy. In this study, RVG‐circDYM‐extracellular vesicles (RVG‐circDYM‐EVs) were engineered target preferentially transfer circDYM brain, effect on process chronic unpredictable stress (CUS) mouse model depression was investigated. The results showed that RVG‐circDYM‐EVs successfully purified by ultracentrifugation from overexpressed HEK 293T cells, characterization demonstrated terms size, morphology specific markers. Beyond demonstrating proof‐of‐concept an RNA drug technology, we observed systemic administration efficiently delivered alleviated CUS‐induced depressive‐like behaviours, discovered notably inhibited microglial activation, BBB leakiness peripheral immune cells infiltration, attenuated astrocyte disfunction induced CUS. CircDYM can bind mechanistically transcription factor TAF1 (TATA‐box binding protein associated 1), resulting decreased expression its downstream genes with consequently suppressed neuroinflammation. Taken together, our findings suggest extracellular vesicle‐mediated treatment clinical applications.

Depression is considered a major public health concern, where existing pharmacological treatments are not equally effective across all patients. The pathogenesis of depression involves the interaction complex biological components, such as immune system and microbiota‐gut‐brain axis. Adjunctive lifestyle‐oriented approaches for depression, including physical exercise special diets promising therapeutic options when combined with traditional antidepressants. However, mechanisms action these strategies incompletely understood. Accumulating evidence suggests that specific dietary regimens can modulate both gut microbiota composition. Here, we review current information about to alleviate their crosstalk inflammatory microbiome. We further discuss role axis possible mediator adjunctive therapies through mechanisms. Finally, future manipulate potential use exercise, interventions, prebiotics/probiotics, fecal transplantation.
