Decades
of
research
have
demonstrated
that
diabetes
affects
racial
and
ethnic
minority
low-income
adult
populations
in
the
U.S.
disproportionately,

patterns

Heart
failure
is
a
clinical
syndrome
that
affects
>6.5
million
Americans,

an
estimated
550
000
new
cases

year.
