Objectives: Evaluate the prevalence of genetic factors in a large population infertile subjects and define seminological, hormonal, ultrasonographic features for each alteration. Methods: This single-center retrospective study included male partners couples undergoing investigations due to oligozoospermia or azoospermia evaluated from January 2012 2022. The consist karyotype, CFTR gene mutations plus variant IVS8-5T polymorphic trait, Y chromosome microdeletion, Next Generation Sequencing panel analyze genes implicated congenital hypogonadotropic hypogonadism (CHH). Results: Overall, 15.4% (72/466) patients received diagnosis cause infertility. Specifically, 23 (31.9%) harbor gene, 22 (30.6%) have 47, XXY 14 (19.4%) show 7 (9.7%) structural chromosomal anomalies, 6 (8.3%) CHH. 80.6% were azoospermic 19.4% oligozoospermic (sperm concentration 3.5 ± 3.8 million/mL). Abstract Infertility is a global issue, with male factors contributing to approximately 50% of cases. While assisted reproductive technologies (ART) resolve 30%–50% cases, 2%–5% IVF failures require rescue intracytoplasmic sperm injection (RICSI). Sperm glycocalyx critical for capacitation and fertilization, sperm‐derived exosomes, essential cell communication, remain underexplored. This study used lectin microarrays fluorescence histochemistry analyze glycopatterns in exosomes from RICSI groups, assessed motility glycan expression after exposure IVF‐derived exosomes. Clinical data 2023 at Northwest Women Children's Hospital, China, showed that couples had longer infertility durations, younger ages, required lower gonadotropin doses, retrieved more oocytes, despite quality. However, pregnancy rates were similar between groups. Glycopattern analysis revealed αGalNAc, Tn antigen, GalNAcα1‐3(Fucα1‐2)Gal (DBA‐recognized) glycans downregulated sperm, while Galβ1‐3GalNAcα‐Ser/Thr (T), GalNAcα‐Ser/Thr (Tn), sialyl‐T (Jacalin‐recognized) upregulated compared IVF. Exosomes higher PHA‐L‐, SBA‐, UEA‐I‐bound glycans, but AAL‐bound than sperm. DBA primarily bound the neck, acrosomal region, plasma membrane, tail its binding was significantly reduced Functionally, enhanced dose‐dependent manner by upregulating DBA‐recognized glycans. These findings provide insights into biomarkers, optimization, glycosylation's role exosome‐related infertility, suggesting exosome‐based therapy improving motility.
