Cutaneous lupus erythematosus (CLE) is an autoimmune disease, which can be limited to the skin or associated with systemic (SLE). Gene expression analysis has revealed that both innate and adaptive immune pathways are activated in CLE. Ultraviolet (UV) light, predominant environmental factor CLE, induces apoptosis keratinocytes, endogenous nucleic acids released from apoptotic cells recognized via pattern recognition receptors, including Toll-like receptors. This leads production of type I interferon, a major contributor pathogenesis by plasmacytoid dendritic cells. Drug-induced lupus (DIL) usually presents after starting a medication known to induce DIL. However unusual presentations are rare, as such, our patient presented with initial signs and symptoms of pericarditis. Once treated he progressively declined angioedema worsening cardiopulmonary status. On first admission, the chest pain that was worsened by laying down improved sitting up. CT Angiography (CTA) showed mild pericardial effusion, EKG diffuse ST elevation, both suggestive pericarditis, for which discharged on colchicine. The readmitted one day later swelling neck tongue. re-evaluated, tested autoantibodies, found positive antinuclear antibody (ANA) suggesting diagnosis lupus, most likely due hydralazine. We report rare presentation drug-induced initially presenting pericarditis evolved into has not been reported in literature thus far. Pericarditis may be lupus. Antinuclear anti-histone antibodies highly sensitive specific respectfully Early time-appropriate discontinuation offending agent patients can life-saving.
