Background
Autoimmune
polyendocrine
syndrome
type
2
(APS-2)
is
a
rare
disorder
characterized
by
autoimmune
damage
to
multiple
endocrine
glands
and
typically
involves
primary
adrenal
insufficiency
(PAI),
thyroid
disease
(AITD),
1
diabetes
mellitus
(T1DM).
Clinical
presentations
that
feature
the
full
triad
alongside
crisis
(AC)
are
rare,
with
only
four
such
cases
reported
globally.
While
AC
most
life-threatening
acute
complication
of
APS-2,
its
pathogenesis
complex
incompletely
understood.
there
potential
triggers,
role
exogenous
substances
as
traditional
Chinese
medicine
[TCM]
has
not
been
systematically
examined.
Case
presentation
A
69-year-old
female
was
hospitalized
9-year
history
increasing
fatigue,
which
had
recently
worsened
due
high
fever,
anorexia,
vomiting
lasting
days.
She
previously
diagnosed
T1DM
(nine
years
prior)
AITD
(five
prior).
Four
earlier,
she
underwent
thymoma
resection.
Three
before
admission,
self-administered
an
unknown
TCM
remedy
coincided
increased
fatigue
mucocutaneous
hyperpigmentation.
On
in
hypovolemic
shock
severe
hyponatremia
(118.0
mmol/L).
Laboratory
tests
revealed
low
basal
cortisol
(2.38
μg/dL)
markedly
elevated
adrenocorticotropic
hormone
(>1250
pg/mL).
An
stimulation
test
confirmed
non-responsive
function,
indicating
PAI.
Together
her
medical
positive
antibody
profile,
APS-2
diagnosed.
responded
well
high-dose
intravenous
glucocorticoid
therapy,
sodium
supplementation,
symptomatic
management.
Although
persistent
recurred
following
discharge,
it
resolved
fludrocortisone
acetate
condition
remained
stable
at
last
follow-up.
Conclusion
We
report
fifth
case
full-triad
document
diagnostic
delay
non-specific
symptoms
asynchronous
multi-glandular
involvement.
Thyroxine
replacement
therapy
TCM-induced
changes
may
have
aggravated
metabolism
immune
imbalances,
hastening
failure.
Clinicians
should
implement
stepwise
organ-function
monitoring
patients
any
single-gland
disease,
maintain
vigilance
for
medication
use,
multidisciplinary
management
strategies
mitigate
risk
AC.
This
provides
critical
insights
into
both
clinical
APS-2.

The
accumulation

the

pharmaceutical
chemical
in
environment

Indomethacin
is
a
non-steroidal
anti-inflammatory
drug
that
potentially
causes
systemic
and
developmental
toxicity
various
tissues.
However,
there
have
been
few
studies
for
its
potential
effects
on
cardiac
development.
In
this
study,
we
systematically
determined
cardiotoxicity
acute
indomethacin
exposure
zebrafish
at
different
concentrations
with
morphological,
histological,
molecular
levels.
Specifically,
malformation
dysfunction
development,
including
pericardial
oedema,
abnormal
heart
rate,
larger
distance
between
venous
sinus
bulbus
arteriosus
(SV-BA),
enlargement
area,
aberrant
motor
capability,
