Background In contrast to existing evidence focusing on high-income countries, this study offers novel insights into the demographic and geographical context that have yet be explored in literature. Comparatively, Pakistan, cognitive impairment is one of neglected disorders can develop dementia Alzheimer's disease. As no treatment available, lifestyle modifications are a valid intervention for health. Methods From January June 2023, cross-sectional recruited 462 participants aged 65 above. We used proven tools gerontological research, such as MIND quiz Quick Physical Activity Rating scale (QPAR), evaluate activity levels. Cognitive function was assessed using Mini-Mental State Examination. Results Our analysis revealed 26.40% had mild impairment, whereas 48.50% demonstrated low diet. The mean QPAR score 20.51 ± 18.77. A significant association found lower levels increased (adjusted odds ratio 9.94, confidence interval (CI): 6.07–16.27). Additionally, higher correlated with reduced (odds 0.29, CI: 0.18–0.46).
